Registration for our October 2026 Canadian Adult Support Group Pilot is now closed.

Adults aged 18 and older living with FND across Canada are welcome to complete the form below to express interest in a future group.

Submitting this form places you on our waitlist. It does not confirm enrollment or guarantee placement in a future group.

HHFFND will contact individuals as future group opportunities become available.

Group Participation Agreement

By submitting this form, prospective participants acknowledge and agree to the following group participation expectations:

  • This group is strictly nonclinical peer-support, not counselling, psychotherapy or medical treatment
  • Respect the privacy and confidentiality of others
  • Speak from personal experience rather than offering advice
  • Allow space for balanced participation
  • Follow facilitator guidance if redirection is needed
  • Maintain respectful and non-judgmental communication

Healing Horizons for FND reserves the right to pause or discontinue participation if group guidelines are not followed, in order to protect the safety of the space.

HHFFND asks every participant to protect the privacy of others. However, HHFFND cannot guarantee that another participant will keep information private.

Participant Agreement

Privacy: HHFFND uses the information submitted through this form to review eligibility, manage the waitlist, and contact applicants about future peer support opportunities. Submitting this form does not guarantee placement in a group. If you do not participate in a group, your information will be retained for up to 12 months after your submission or last contact. If you participate, your information will be retained for the duration of your participation and two years afterward.
